FATAL ERROR status on I/O commands
Cause and prerequisitesIt appears that the SMART Controller resets the SCSI bus and does not resume operation properly. Then it returns a fatal error status on the calling driver.
The SMART Controller may occasionally retrun FATAL ERROR status on I/O commands under circumstances that may be recoverable.
This is NOT a RAID failure! RAID is still doing its job, but the communication between the disks/controller/driver/OS will be impaired resulting in the above mentioned scenarios.
In order to protect the data, upgrade the SMART CONTROLLER firmware and the ProLiant System firmware as follows:
SMART SCSI-2 Controllers:
Firmware version 2.22 found in Option ROMPaq V. 2.24 Rev A (SP1318)
ProLiant 2000/4000:
System ROM version 07/18/95, found in ProLiant 2000 and 4000 System ROM (18 July 1995) Ver. 18 July 1995, E10 (SP1305)
ProLiant 4500:
System ROM version 06/23/95, found in ProLiant 4500 System ROM (23 June 1995) Ver. 23 June 1994, E14 (SP1298).
PROACTIVELY UPGRADE ALL SMART CONTROLLERS TO FIRMWARE REV. 2.22 IMMEDIATELY!!
At the same time upgrade the ProLiant 2000/4000/4500 to the above mentioned versions.
If a newer version of the firmware is available at the time of installation, please use that.
