SAP Note 19456 - ORACLE: Startup fails: "File already exists"

Symptom:

Startup of DB fails ==> File already exists

Cause and prerequisites

When a crash occurs, the $ORACLE_HOME/dbs/sgadef.dbf file gets stuck.

Solution
Rename the file and restart the database.
